Steps to Accessing Services/Supports from Division of Developmental Disabilities (DDD)
DDD has developed an informational handout for families of students with intellectual and developmental disabilities who will exhaust their educational entitlement when they turn 21 years of age. DDD’s Timeline for Students Exiting School and Turning 21, commonly known as the Graduates Timeline, provides an overview for young adults and their families who wish to pursue Division-funded services after age 21.
Transition Services should:
- be a part of the Individualized Education Program (IEP).
- be based on a student’s individual preferences and interests.
- begin at age 14 (or earlier if applicable).
- be a long-range plan that emphasizes skills that are needed for adult life.
